The election takes place in the Landtag.
Under Article 52, the Landtag elects the Minister President by secret ballot from among its own members. The first ballot requires more than half the statutory membership. Voters therefore elect the Landtag; they do not appoint the head of government through a separate direct election.
After the election, the head of government appoints the ministers and sets policy guidelines. Within those guidelines, each minister manages their own department independently and under their own responsibility. Government leadership does not remove that division of duties.

The other three answers
The other titles do not fit this state office.
First Minister
First Minister is not the title used in NRW’s constitution. The title here is Minister President.
Prime Minister
Prime Minister is not the German constitutional title of this office. Minister President is the answer required by this question bank.
Mayor
Mayor is a municipal office. Leading a town or municipality is different from leading NRW’s state government.
Presiding over parliament and leading government are different offices.
The Landtag also elects its own presidency. The President of the Landtag does not thereby lead the state government. The head of government has a separate title and is elected through a separate procedure.
If the required majority is not reached on the first ballot, Article 52 provides for further ballots and, if necessary, a runoff. For the test question, the office title is enough; it does not ask for the current officeholder’s name.
Articles 51–55 · State government · Constitution of North Rhine-Westphalia ↗NRW state government → Minister President.
