Citizens elect parliament; parliament elects the head of government.
The Landtag elects the Minister President by a majority of its statutory membership. The office is therefore not filled through a separate direct election by all eligible voters. The parliamentary election and the election of the head of government are connected but remain distinct steps.
The head of government chairs and directs the government’s work and sets its political guidelines. They appoint and dismiss ministers and other government members with the Landtag’s consent. Within those guidelines, ministers run their portfolios independently.

The other three answers
Saarland’s specific office title is what matters.
First Minister
First Minister is not the title used in Saarland’s constitution. A similar general meaning does not make it the required test answer.
Prime Minister
Premierminister is not the German office title sought here. In English, prime minister can sometimes be used as a translation depending on context; this question bank expects Minister President.
Mayor
Mayor is an office at the municipal level of a town or municipality. The question concerns the government of the entire federal state instead.
Government leadership depends on parliamentary confidence.
Article 88 requires government members to enjoy the Landtag’s confidence. Withdrawal of confidence follows a constitutional procedure and requires a majority of the statutory membership. The head of government cannot therefore govern independently of parliament.
The question asks for the office title, not the current officeholder’s name. The President of the Landtag is also a different office, belonging to the leadership of parliament.
Articles 87, 90 and 91 · Constitution of Saarland ↗Saarland state government → Minister President.
Parliament and government are connected, but their leadership offices remain distinct.
