Voters elect the Landtag; the Landtag elects the head of government.
The Landtag elects the Minister President by secret ballot without debate. The first two ballots require a majority of its members. If no such majority is achieved, Article 70 states that the candidate receiving the most votes is elected in the further ballot. This is Thüringen’s own constitutional rule.
The state government consists of its head and the ministers. Article 76 combines the Minister President’s power to set policy guidelines with ministers’ own responsibility for their departments. Leadership therefore does not mean personally taking every administrative decision.

The other three answers
Distinguish the German title from an English translation.
First Minister
First Minister is not the office title used by Thüringen’s state constitution.
Prime Minister
Premierminister is not the German title here. Prime Minister can, however, appear in English as a translation of a German Ministerpräsident. In these options, the catalogue expects Minister President.
Mayor
A mayor heads a municipal level of government. Thüringen’s state government is led by the Minister President.
Head of government is not president of parliament.
The Landtag president leads parliament; the Minister President leads the government. The similar-sounding titles describe different functions within the separation of powers.
Under Article 73, the Landtag can express no confidence only by electing a successor with a majority of its members. This constructive no-confidence procedure combines removal with replacement leadership.
