The title identifies leadership of the state government.
The Minister-President sets the government’s policy guidelines and is accountable to parliament for them. Within those guidelines, ministers run their respective departments independently and under their own responsibility.
The head of government also chairs the state government. This does not confer unlimited unilateral decision-making: the cabinet decides under constitutional rules, and parliament has its own legislative, budgetary and scrutiny functions.

The other three answers
The German title is clear; English translations need extra care.
First Minister
First Minister is not the title used for Brandenburg’s head of government in this question. Choose the specified title Minister-President.
Prime Minister
Prime Minister can be an English translation of the same office: an official state website uses State Prime Minister. However, the unchanged English quiz marks Minister-President as correct. The German alternative Premierminister is not the constitutional title.
Mayor
Mayor is a local-government office. The government of Brandenburg state as a whole is headed by a Minister-President.
Distinguish the office, its holder and its translation.
The question asks for a title, not a person or party. Even when government personnel change, the constitutional term remains relevant. The president of the Landtag holds a different office: leading parliament.
Outside this set of choices, Minister-President and Prime Minister can identify the same head of government. The knowledge check preserves the existing choices and explains this translation overlap rather than turning it into two distinct Brandenburg offices.
Articles 82–84 and 89–90 · State government · Constitution of Brandenburg ↗Brandenburg’s state government → Minister-President.
In the English quiz choose Minister-President; in German, use the constitutional title.
