The Senate governs; the Bürgerschaft is parliament.
Hamburg’s Bürgerschaft elects the head of government by a majority of its statutory membership. The elected person then appoints the Second Mayor and the other senators and seeks the Bürgerschaft’s confirmation. The population therefore does not elect the First Mayor in a separate direct election.
The First Mayor directs the Senate’s business and sets political guidelines, remaining accountable to the Bürgerschaft for them. The Senate as a whole directs and supervises the administration. The distinctive title does not change the fact that Hamburg has a state government accountable to parliament.

The other three answers
Not every similar political title identifies Hamburg’s head of government.
Minister President
Minister President is the usual title for a territorial state’s head of government. Hamburg’s constitution instead uses First Mayor.
Governing Senator
Senators do belong to Hamburg’s government. Governing Senator, however, is not the title prescribed here for the head of government.
Lord Mayor
Lord Mayor is not the requested Hamburg title. First is part of the correct title and should not be replaced with Lord Mayor.
The question asks for the office’s title, not a person’s name.
The officeholder can change. The answer remains the title used in the constitution. A change during an ongoing parliamentary term is also possible: the Bürgerschaft can elect a successor with the required majority, thereby withdrawing confidence from the previous head of government.
The President of the Bürgerschaft, by contrast, presides over parliament. This is a different office from President of the Senate. Always check the institution following the title: Bürgerschaft refers to parliament, Senate to government.
Articles 33, 34 and 42 · Senate and head of government · Hamburg constitution ↗Hamburg: First Mayor → head of the Senate.
The Bürgerschaft elects the person. The Senate is the state government.
